You must meet ALL requirements below to be eligible to join us for FREE:

1: Must be 18 years old or above

2: Must be a Medi-cal holder

The California Medical Assistance Program (Medi-Cal or MediCal) is California's Medicaid program serving low-income individuals, including families, seniors, persons with disabilities, children in foster care, pregnant women, and childless adults with incomes below federal poverty level. 

3: Currently enrolled in one of the following managed Care Plan (HMO, PPO, POS) from one of the following insurance providers:

• Molina

• LA Care

• Scan

• ​IEHP

• Health Net

• Kaiser Permanente

• Anthem Blue Cross

• BlueShield

4: Individual who as functional impairments, either physical or mental, for the purpose of restoring or maintaining optimal capacity for self-care. 

• Has 2 or more diagnoses such as Hypertension, Diabetes, Insomnia, Alzheimer, Hyperlipidemia, Benign prostatic hyperplasia, Depression, Anxiety, Gastroesophageal Reflux Diseases, etc.

• Physically isolated and lack of sufficient social support

• Hospitalization or ER visits within past 12 months
